Persoonlijk Zorgnetwerk: the client in control

A web application for collaboration and decision-making in care for children and young people.

Commissioned by Oog op Zorg and Huis voor de Zorg, we developed the Persoonlijk Zorgnetwerk (Personal Care Network). The application is built for complex situations, in which a family is dealing with several forms of care and support at the same time. The client is in control, keeps the overview and is at the centre.

One child, one family... and a whole series of care professionals

Around a family in a complex situation, several forms of care and support are often active at the same time. Each with its own file, its own perspective and its own vocabulary. Those who need the overview most usually have it least: the family itself.

The starting point

The client is in control

PZN turns that relationship around. The client is at the centre, keeps the overview and decides who looks in and who has a say. Digital tools make new forms of collaboration possible; a shared language, openness, overview, control and independence of place and time form the core.

Control

The client decides who looks in

The care network belongs to the client, who keeps the overview of who is involved and is at the centre of decision-making, rather than hearing about it after the decision has been made.

Shared language

One picture everyone can point to

Life domains of child, family and environment come together in one model. That gives everyone involved a shared language: you discuss the same situation in the same terms, even when you come from a different discipline.

Collaboration

Independent of place and time

Those involved do not need to be in the same room at the same time to reach a decision. Openness and overview are built in, so collaboration does not depend on who happened to be at the meeting.

Origin

Born between youth care and paediatrics

PZN grew out of an initiative between youth care and paediatrics and was developed further in the Parkstad region, where general practitioners are also involved.

The foundation

PZN, too, was built with Polaris

The application was developed with Polaris, our software generator. Precisely in a domain concerning children and families, the standard layer underneath is no side issue: who may see what, who changed what, and how that remains demonstrable. That comes from the foundation rather than from manual work per project.
